Consent to the processing of personal data and the use of video reviews

By submitting my video review via this form, I grant the company
BEWIT Natural Medicine s.r.o., ID No. 29443237, with registered office at Michalská 2030, Ostrava, Czech Republic
(hereinafter referred to as "BEWIT"), as the data controller, my explicit consent to the processing and use of the video review sent by me (including my voice, likeness, name and other personal data expressed), namely:

1. to present and promote BEWIT's products, brand and activities,
2. on BEWIT's website, social networks, advertising materials, presentations and other communication channels,
3. without time and territorial limitation.

I give my consent voluntarily and I am aware that I can withdraw this consent at any time by sending a request to the following e-mail address: [email protected].
However, the withdrawal of consent shall not affect the lawfulness of processing carried out before its withdrawal.

I certify that I am the author (or authorised user) of the submitted video and that its content does not infringe the rights of third parties. I agree that BEWIT may modify, shorten or add to the submitted material (e.g. by adding subtitles or graphic elements), while preserving its meaning and dignity.
